That’s the verdict handed down by the Canada Task Force on the cause of the massive August 14, 2003, power blackout that left some 50 million people in Canada and the US in the dark.
The blame was placed at the door of MSBlast, an e-worm that exploited a Windows vulnerability and which infected millions of machines.
But institutional, human, and computer error were the actual culprits, says the task force.
“The August 14 blackout – and every major regional blackout that preceded it dating back to 1965 – could have been prevented if clear, common-sense reliability measures had been followed,” says US USĀ Federal Energy Regulatory Commission chairman Pay Wood, III.
