The biggies automatically get mucho play but sites such as p2pnet are lucky for a look-in. Inclusion depends on arcane algorithms which apparently change frequently. Time of posting relative to when a story breaks also seems to be important, as does whether or not the mainstream media see something as 'news' and jump on it.
